What to Expect and What to Bring

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

New Patient Registration

You may pre-register with our office by filling out our online Patient Registration Form. When you call the office for your appointment, you will be provided the registration code needed to complete this process.

Once you have that code, click on the "Patient Login" button which will take you to a secure login page. On that page, enter the registration code you were provided.

On your first visit to our office, we will review your form and ask you to sign. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Bring to Your Visit

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
  • X-rays taken in the last 6 months
  • IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.

Your Comprehensive Evaluation

Your first visit includes a comprehensive exam in which our doctor will talk to you about your dental and medical history and note any questions or concerns you may have. Many health conditions are indicated in the mouth and can affect oral health, so your medical information is very important in diagnosing issues with your teeth, gums and mouth.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for pre-med.)
